6 Year Experience of Percutaneous Intervention of Chronic Total Occlusion at YUMC

Percutaneous revascularization of chronic total occlusion (CTO) remains difficult despite advances in angioplasty technology and operator experiences. Recently, coronary stenting has emerged as a valuable therapeutic strategy for the management of CTO. To elucidate initial success rate and angiographic follow up, a consecutive 79 patients (79 lesion), who underwent procedure between June, 2000 and June, 2005 in our institution were investigated. Mean age of the patients was 57.9±9.3 (36-79, M:F = 67:12) years. The locations of the lesion were left anterior descending (n=35, 44.3%), right coronary (n=34, 44.0%), and left circumflex (n=10, 12.7%), respectively. Most lesions were de novo (n=60, 93.7%), whereas in-stent restenosis lesions were present (n=4, 6.3%). Procedural success was achieved in 64 lesions (81%) and all the 64 lesions underwent coronary stenting. The type of stents were divided by drug-eluting stents (n=75, 82.4%) and bare metal stents (n=16, 17.6%). Mean diameter of the stents was 2.97±0.45 mm (2.5-4.0) and mean length of the stents was 27.64±6.17 mm (13-36). The technical reason for procedural failure was the inability to cross the lesion with the guide wire (15 lesions), and failure to cross the lesion or dilate it with a balloon accounted for 4 technical failures. Neither procedural death nor the necessity for emergency coronary artery bypass surgery occurred. Angiographic data at 6 months were available for 25 lesions (39 %). Although angiographic follow-up has not been completed, the restenosis rate after successful dilatation of CTO was 12 %. Mean follow up duration was 15.13±14.9 months and there were no other major cardiac events, including cardiac death and acute myocardial infarction in all the patients. In properly selected cases, the success rate with standard wire technique of PCI for CTO is acceptable.
