Severity Measurement of Degenerative Changes on Heart Structure with New Method and Its Relation with Coronary Artery Calcification

BACKGROUND: There has been no study about severity measurement of degenerative change on heart structure. PURPOSE: To investigate the relation of coronary calcification(CC) and degree of cardiac degeneration severity measured by image processing software program. METHOD: The study consisted 88 patients (44 male, mean age 55±11yrs) who showed degenerative change of aortic or mitral valve(AV or MV), aortic root(AR) or mitral annulus(MA) on echocardiography. The gray scale(GS) of echocardiographic images of AV and MV, AR, MA were analyzed with Adobe photoshopTM software and expressed as an ratio to normal myocardial GS. The amount of CC on coronary multidetector CT scan was quantified according to the Agatston score system. RESULTS: The GS of AR was one of the independent predictors of CC (Odds ratio=1.29, p=0.02). GS of AR showed positive correlation with the amount of CC. (r=0.572, p<0.01)(Figure 1) At the cut off value of AR GS 3.25, CC was predictable with 67% sensitivity and 70% specificity.(area=0.71, p<0.01) (Figure 2) CONCLUSION: Measurement of cardiac degenerative changes with image processing soft ware is feasible. AR GS calculated with this method have positive correlation with the amount of CC and is a predictable parameter for CC at the cut off value of 3.25.
