Brugada like ECG changes in high intercostal space in patients with incomplete RBBB without syncope and dizziness

Background: Distinctive characteristics of Brugada syndrome is incomplete RBBB(ICRBBB) with ST segment elevation in right precordial leads. When patients with syncope or dizziness have these ECG characteristics, it suggests Brugada syndrome(BS). When these changes are shown in high ICS with symptoms, it also suggests BS. But we frequently found non-cardiology patients with ICRBBB showed ST segment elevation and T wave inversion in high ICS. Because of that, we think, ECG changes in high ICS is non-specific findings. So we want to evaluate ECG changes in high inter-costal space in patients with ICRBBB without dizziness or syncope. Method: Patients with rSr’in V1 were tested for high ICS ECG and classified for BS ECG type 1, 2, and 3; also measured their J point amplitude, T wave, ST-T configuration. Examines’ clinical characteristics were evaluated also. Evaluation of ECGs were done by two qualified investigator. Result: 30 patients with ICRBBB were evaluated 4th, 3rd and 2nd ICS ECG(group A ) and for 32 controls (group B ). ECGs were classified ICRBBB, BS type 1, type2, type3. In 3rd ICS, 1 patient (3.1%) showed type 1 and 12 patients (40.0%) showed type 2 and 3 ECG. In 2nd ICS, 2 patients (6.2%) showed Type 1 and 18 patients (60.0 %) showed type 2 and 3. Conclusion: In our study, patients with ICRBBB did not have syncope and dizziness, but they showed BS like ECG changes in high ICS. There were significant elevations of J point in high ICS. Because there were no patients complained BS like symptoms. Brugada like ECG changes in high ISC do not suggest high risk of sudden cardiac death in general people. Future large scale trial for high ICS ECG changes will be needed
