Outcomes of Drug-Eluting Stent Implantation for Treatment of Diffuse Long Lesions: an Update of Korean Multicenter Angioplasty Team(COMATE) Registry

Background. Diffuse long coronary lesion (over 30mm) is a well-known predictor of ominous outcomes after stent implantation. With the implication of drug-eluting stents (DES), outcomes were greatly improved even these lesions and we previously reported that. This is an update of COMATE registry for the diffuse long lesion. Method and Results. From April 2003 to January 2005, 458 diffuse long lesions in 402 patients were treated with DES implantation {360 sirolimus-eluting stent (SES), 98 paclitaxel-eluting stent (PES)}. Reference diameter was 2.79+/-0.49 mm and minimal lesion diameter (MLD) was 0.62 ± 0.46 mm. All the cases were successful. Post-procedural MLD was 2.41 ± 0.71 mm. Angiographic follow-up was done in 145 patients (36.1%; SES 112/360, 31.1%: PES 33/98, 33.7%). There was 10 in-stent restenoses (6.9%, SES 8/112, 7.1%, PES 3/33, 9.1%, NS). Follow-up MLD was 2.41 ± 0.71 mm with late loss of 0.33 ± 0.55 mm. Of 402, 17 patients experienced major adverse cardiac events (MACE, 4.2%) including 9 deaths. Two patients had non-fatal myocardial infarction due to subacute thrombosis within 30 days. Six more patients underwent target lesion revascularization (8/458, 1.7%). Conclusion. In our registry data, diffuse long lesions treated by DES implantation showed excellent angiographic results and very low MACE. More detailed data with added registry will be presented.
