Remission of vasospastic angina among Korean patients in angiography within a period of several years

Object: The pathogenesis of vasospastic angina is not completely understood, but endothelial dysfunction and smooth muscle hyper-reactivity are known to be involved. Differences in the nature of vasospasm were reported between Western and Eastern vasospastic patients. This study regards to among Korean patients diagnosis with variant angina, as follow up angiography to see on location, condition and remission of spasm in several years later. Methods: Subjects were 493 patients who underwent positive coronary spasm tests from January, 1995 to December, 2003 at the Kangnam St. Mary’s hospital. We reexamined 37 patients (62 lesions) with coronary angiography and acetylcholine provocation test. Changes in ECG(ST elevation), along with chest pain, atheroma were 50% less in coronary artery, and spasm show more than 90% in Acetylcholine provocation test, this is positive on spam studies methods. Remission of coronary vasospasm did not show any chest pain while patients did not prescribe medication and negative on provocation test with normal angiography. Results : The mean interval from diagnosis to follow-up angiography was 39±14 months. The mean age was 52±6.3 years, and 23 of them were male. None of the 62 lesions had progression of atherosclerosis on QCA. All of them showed vasospasm at the same original location. Nine patients showed new vasospastic lesions. Conclusion : Angiographic follow-up of 37 Korean patients with vasospastic angina at a mean of 3 years revealed vasospasm at the same original location in all of the patients and no remission was seen.
