Relation between brachial-ankle pulse wave velocity and framingham risk score.

Objective The Framingham risk score(FRS) evaluates the probability of developing CHD over next 10 years. Over 20% of FRS is considered as coronary heart disease equivalents. It is known that aortic pulse wave velocity(PWV) is related with FRS and cardiovascular mortality, but the relation between brachial-ankle PWV(baPWV) and FRS is not well established yet. Purpose of this study is to evaluate the applicability of baPWV for screening cardiovascular risk and to determine the level of baPWV to select high CHD risk group among subjects who has at least one cardiovascular risk factor. Design and Methods This work is designed as a cross sectional study with 1044 asymptomatic adults(53.7±13.1 years old,male 52.5%) with 1 or more coronary risk factors. We got the FRS and baPWV of all subjects. Result baPWV is well correlated with FRS (r=0.447 ,p<0.001). baPWV is an independent risk factor to predict cardiovascular disease(p<0.001). To screen high CHD risk group(FRS>20%), the cut-off baPWV value is 1649 cm/sec(Sensitivity 61.9%,Specificity 74.3%). And upper quartile(1820 cm/sec) of baPWV is the most potent predictor of CHD(odds ratio 6.8) over other conventional risk factors (age, blood pressure, gender, diabetes, smoking,total cholesterol level).Conclusion baPWV is strongly associated with FRS and a potent surrogate marker of predicting future coronary event risk in asymptomatic patients with at least 1 coronary risk factors.
