Clinical and Angiographic Outcome of Drug-Eluting Stent implantation for Treatment of Diffuse Long Lesion: Korean Multicenter Angioplasty Team(COMATE) Registry

Background Coronary lesion length is a predictor of adverse outcomes after bare metal stent implantation. The remarkable efficacy of drug-eluting stent(DES) has lead to their use for treatment of diffuse disease. The objective of this study was to assess the value of DESs including sirolimus-eluting stent (SES) and paclitaxel-eluting stent (PES) implantation in patients with diffuse long lesion over 30mm of total stent length including overlapping stent. Methods Between April 2003 and April 2004, 195 patients with diffuse long lesion were treated with implantation of 1 or more DESs (259 SES and 79 PES). Six-month clinical and angiographic outcome were evaluated. The primary end point was the incidence of major adverse cardiac events (MACE; Death, myocardial infarction[MI] and target-lesion revascularization[TLR]). The secondary end point was angiographic restenosis (≥50% diameter stenosis at follow-up angiography). Results Pre-procedural reference diameter was 2.87±0.45mm. Minimal lesion diameter (MLD) was 0.51±0.70mm. Post-procedural MLD was 2.71±0.44mm. At six-month clinical follow-up, the MACE rate was 3.5% (7 patients; 4 death, 3 TLR [2 SES, 5 PES]). Six-month angiographic ISR was 10.5% (8/76 lesions; 5 SES, 3 PES). The late loss was 0.51±0.70mm (0.43±0.66mm in SES, 0.69±0.78mm in PES). Even in extreme long stenting over 60mm, so called, ‘metal jacket’ cases (15 patient; 13 SES, 2 PES), late loss was 0.69±0.77mm and all cases revealed patent results. Conclusion Drug-eluting stent implantation is considered safe and efficacious in the treatment of diffuse long lesion. More angiographic and clinical follow-up data will be showed at Congress of The Korean Society of Circuation.
