Recovery Pattern and Term of Reversal of Left Ventricular Remodeling in Patients with Congestive Heart Failure

Background: Reversal of heart failure remodeling has been observed with intensive congestive heart failure(CHF) therapy. Recovery pattern and terms in CHF patients with reversible left ventricular systolic dysfunction(RLVSD) has not been elucidated. Subjects and Methods: We analyzed the clinical recovery findings in 88 CHF patients with RLVSD from consecutive 592 patients with severe LV systolic dysfunction(LVED>60mm and LVEF<30%) admitted in Hallym University Hospital during the period of Sep 1999 to Jul 2003.RLVSD was defined as decreased LV chamber size and improved LVEF above 40%. Results: Mean follow up period was 35±16 months. RLVSD occurred 15% in patients with severe LV systolic dysfunction. Underlying disease of RLVSD were ischemic cardiomyopahty(I-CMP), hypertensive CMP(HT-CMP), tachycardia induced CMP(T-CMP) and idiopathic dilated CMP(D-CMP), other disease include in 4 cases of alcoholic CMP , 1 case of postpartum CMP and after aortic valve replacement. Results of recovery pattern and term showed in table. *LVEF>40% and LVED>54mm, **LVEF>50% and LVED≤53mm Conclusion: RLVSD was not rare in CHF patients with severe LV systolic dysfunction and also different underlying disease had similar recovery pattern except T-CMP, but recovery term depended on underlying heart disease.
