Pulse Wave Velocity in Upper- and Lower-Limb Arteries among Patients with Peripheral Arterial Disease

Background: Peripheral artery disease (PAD) is one of the major complications of atherosclerosis. Arterial Stiffness is a key components of atherosclerosis and can be assessed by measuring physiological properties with pulse wave velocity (PWV). There is only a limited amount of information regarding possible changes in PWV among patients with PAD Subject and Method: We measured heart-right brachial PWV (hbPWV) and right and left brachial-ankle PWV (RbaPWV and LbaPWV) using automatic wave form analizer (model VP-1000, Colin, Japan) in 34 hypertensive patients with PAD and 33 hypertensive control paitients without PAD. Results: There was no significant difference between the PAD group and control group in age, gender and body mass index. Hypertensive patients with PAD showed a higher hbPWV (644 ± 113 cm/sec, p<0.05) than the patient without PAD (579 ± 110 cm/sec). There were no significant differences in ba PWV between two groups. In contrast, among the patients with PAD, baPWVs were significantly lower in the affected legs than in non-affected legs and the reduction in baPWV was greater in those with lower ankle-brachial index (ABI). RbaPWV (1774 ± 460 cm/sec) and LbaPWV (1801 ± 542 cm/sec) of non-affected legs among the patients with PAD were higher than those (RbaPWV: 1582 ± 441 cm/sec, LbaPWV: 1638 ± 423 cm/sec) of the patients without PAD, but statistically not significant. Conclusions: This results suggest that hbPWV is higher in hypertensive patients with PAD than hypertensive control patients, whereas baPWV is decreased in the affected legs in the patients with PAD. High hbPWV may be a novel maker of Atherosclerosis Obliterance in hypertensive patients.
