Long-term prognosis of vasospastic angina according to degree of vasospasm during intracoronary acetylcholine provocation tests

Object: The long-term prognosis of angiographic equivocal group of vasospastic angina has not been established yet. In this study, we analyzed the long-term prognosis of vasospastic angina according to degree of vasospasm during coronary spasm tests. Methods: Subjects were 156 patients who underwent positive coronary spasm tests from the period January, 2000 to December, 2003 at the Kangnam St. Mary’s hospital. Patients with coronary spasms greater than 90% with ST changes accompanied by chest pain were classified as ‘strong positive’. Patients with coronary spasms, range from 70% to 90% were classified as ‘equivocal positive’. Both groups were retrospectively analyzed with clinical records and phone interviews for mortality, chest pain frequency(1-5 grade), and admission due to chest pain. Results: When the strong positive group(n=113) was compared with the equivocal positive group(n=43), smoking(p<0.001), mean number of vessels with spasm(p=0.02), the percentage showing multivessel spasm (≥2 arteries)(p=0.02) were more frequent in strong positive group. We compared the mortality, admission due to chest pain during the average 26.4±14.1 month follow-up. Although no statistical differences were observed between both group(p=0.12), there were 8 patients(7.1%) of cardiac death in strong positive group as against none in equivocal group. There was 19 patients(16.8%) with all causes of death in strong positive group, but only 3(6.9%) in equivocal group. Admission due to chest pain was higher in the strong positive group(p<0.001). Also the strong positive group showed a statistically significant higher chest pain frequency(p<0.001). Conclusion : The long-term prognosis of angiographic equivocal positive group was significantly better than that of strong positive group. However in strong positive group, chest pain attak was more frequent, and also the trend of cardiac death was observed.
