Tissue Doppler Imaging Derived E/E’, N-Terminal Pro- B-Type Natriuretic Peptide, and Left Atrial Volume Index for Prediction of Prognosis in Patients with Congestive Heart Failure

Background Tissue Doppler Imaging (TDI) derived the ratio of early transmitral flow velocity to early diastolic mitral annulus velocity (E/E’), N-terminal pro- B-type natriuretic peptide (NT pro-BNP), and left atrial volume index (LAVI) have not been directly compared for the prognosis of patients with congestive heart failure (CHF). Methods Patients who admitted for CHF had simultaneous clinical examination, complete echocardiographic examination, and NT pro-BNP. Primary end point was defined as cardiac death or rehospitalization for CHF. Results Seventy-four patients with CHF {ischemic (n=39) or non-ischemic (n=37), 44 males (59.5%), mean age 68.4±10.7 years) were enrolled. The mean follow-up duration was 334±170 days. Fourteen patients (18.9%, ischemic, n=9; non-ischemic, n=4) suffered an event (cardiac death, n=3; rehospitalization due to CHF, n=11). In patients with an event, ejection fraction was lower, mitral deceleration time was shorter, and E/E’, LAVI, and NT pro-BNP were elevated when compared to event-free patients, significantly (Table). Restrictive mitral filling pattern was more frequent in patients with an event than event-free patients (48.3% vs. 20.2%, p=0.015). By multiple logistic regression analysis,, E/E’ was the only significant predictor of adverse outcome. Conclusions In patients with CHF, E/E’ is the most important predictor of future cardiac events.
