Influence of chronic atrial fibrillation on the hemodynamic characteristics in patients with severe mitral stenosis

Background Hemodynamic characteristics are important to assess the disease severity and determine the therapeutic plan in patients with mitral stenosis (MS). Because of loss of left atrial (LA) pump function and progressive LA enlargement, atrial fibrillation (AF) may affect hemodynamic characteristics in MS. Therefore, we assessed hemodynamic influence of chronic AF in severe MS.
Methods and Results We analyzed the clinical, echocardiographic, and cardiac catheterization data from 356 patients who underwent percutaneous mitral commissurotomy (PMC). Although normal sinus rhythm group (N=225) and AF group (N=131) had comparable mitral valve area and functional class, AF group showed lower mean LA pressure, mean mitral gradient, and pulmonary artery systolic pressure, and larger LA volume. Cardiac index was significantly lower in AF group. Despite successful PMC, LA volume reduction and symptom improvement were smaller in AF group (Table 1).
Conclusions Patients with chronic AF showed different hemodynamic features from those with normal sinus rhythm in severe MS. This hemodynamic influence of AF should be considered in evaluating patients with MS.
