Determinants of N-terminal pro-brain natriuretic peptide (NT-proBNP) levels in patients with primary dilated cardiomyopathy |

Background
Measurement of plasma N-terminal pro-brain natriuretic peptide (NT-proBNP) levels has been used for the diagnosis of heart failure. However, major determinants of NT-proBNP in primary dilated cardiomyopathy (DCM) need to be elucidated. This study was aimed to characterize the echocardiographic variables to determine the plasma NT-proBNP levels in patients of primary DCM.
Methods
From the 83 patients with primary DCM without nephropathy (plasma creatinine level<1.5mg/dl), echocardiographic indices and grade of NYHA (New York Heart Association Functional Classification) were compared with plasma NT-proBNP levels which were measured within 2 weeks after echocardiographic examination. The relationship between the NT-proBNP levels and echocardiographic variables were analyzed.
Results
Baseline characteristics of the primary DCM patients are listed in Table 1. In multivariate analysis, LV EF (p=0.027), LVIDs (p<0.001), LVIDd (p<0.001), and tricuspid regurgitation (p=0.002) have significant correlations between ln[NT-proBNP] (p<0.001, R =0.706).
Conclusion
Plasma NT-proBNP level may be related both structural and functional change of LV in primary DCM.
